This section presents the budget and staffing information for the PATH Annual Report. PATH providers must report actual budget values.

The PATH grant supports outreach to individuals with serious mental illness, as well as individuals with co-occurring substance use disorders, who are homeless or at risk of becoming homeless. This section includes data on persons contacted through street outreach and services-only projects and the number of persons enrolled in the PATH program.

This data represents the total number of persons contacted, the number of new persons contacted, and the total instances of contact during this reporting period.

This data represents the total number of persons enrolled in PATH, the number of new persons enrolled, the percentage of people eligible for PATH who became enrolled, and the number of persons who could not be enrolled during this reporting period.

For each demographic element, data represents the number of PATH-enrolled individuals identifying with each response category. Each demographic element will sum to the number of active, PATH-enrolled individuals (Q15) except for race and ethnicity, and prior living situation.

For the Veteran status and domestic violence categories, the element will sum to the number of active, PATH-enrolled adults. If a PATH provider enrolled an individual under age 18, they will not be counted in these elements.

This data represents the number of PATH-enrolled individuals identifying with each response category for age. This data will sub to the number of active, PATH-enrolled individuals for the reporting period. Adults are the primary target population for the PATH program. However, some programs may enroll transition-age youth aged 17 and under if they meet program eligibility.

This data represents the number of PATH-enrolled individuals identifying with each response category for race and ethnicity. Individuals who identify with multiple races or ethnicities are counted in all applicable categories. As a result, this demographic element's total may exceed the number of active, PATH-enrolled individuals.

This data represents the number of PATH-enrolled adults over 18 who identify as Veterans. If a PATH provider enrolled an individual under age 18, they will not be included here.

This data represents the number of PATH-enrolled individuals identifying as having a co-occurring substance use disorder.

This data represents the living situation of each PATH-enrolled individual at the time of enrollment.

Of those identified with a Prior Living Situation as staying in a place not meant for habitation (#26h1) or emergency shelter (#26h2), this data represents the length of time these individuals were in that living situation. This data element will not sum to the number of persons enrolled during the reporting period unless all persons enrolled were staying in a place not meant for human habitation or emergency shelter.

This data represents the number of PATH-enrolled individuals identifying as experiencing chronic homelessness. An individual’s chronic homelessness status is determined based on information entered in HMIS.

This data represents the number of PATH-enrolled adults over 18 who identify as having experienced domestic violence. If a PATH provider enrolled an individual under age 18, they will not be included here.

The PATH program connects individuals with treatment, recovery support services, housing, and income assistance through the direct provision of PATH-funded services or by referring individuals to community support services. Data in this section represents services provided using PATH funds, referrals made to community partners, and services or assistance attained through those referrals. For each category, the data represents the unduplicated number of PATH-enrolled individuals who receive a service or referral. Individuals who received more than one of each type of service or referral will only be counted once in each category.

This table reports the unduplicated total number of enrolled PATH clients who received each PATH service during the reporting period. Individuals who received more than one type of service (e.g., clinical assessment and case management) are recorded once in all service categories that apply.

This data represents the number of unduplicated PATH-enrolled individuals who received at least one referral and those who attained the service due to the referral. Individuals who received more than one referral within a category (e.g., multiple housing services referrals) will only be counted once in the respective category. Individuals who received more than one type of referral (e.g., community mental health and substance use treatment) should be recorded once in each of the referral categories that apply. Referrals provided prior to PATH enrollment are not counted here.

PATH providers support enrolled individuals in obtaining income, mainstream benefits, health insurance, and permanent, safe housing. The data in this section represents the status of each benefit/service for PATH-enrolled individuals at project entry, project exit, and those still active and enrolled at the end of the reporting period.

This data represents the number of PATH-enrolled individuals who had income from any source during the reporting period. Due to the high number of individuals who left the program without completing an exit interview, it is believed that this number may be a potentially significant undercount of those who obtained benefits during their time receiving PATH services.

This data represents the number of PATH-enrolled individuals who received Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI) during the reporting period. Due to the high number of individuals who left the program without completing an exit interview, it is believed that this number may be a potentially significant undercount of those who obtained benefits during their time receiving PATH services.

This data represents the number of PATH individuals connected to SSI/SSDI Outreach, Access, and Recovery assistance during the reporting period.
